  Policy on Confidentiality and Privacy

Floral Clinic Physiotherapy Service - hereby shortened as PC is committed to protecting the confidentiality and privacy of all client information. We have implemented strict security measures to ensure the protection of client information, including physical, administrative, and technical safeguards. Our employees are trained in the importance of maintaining the confidentiality and are required to sign confidentiality agreements.

 1.  Policy Objective

  • This policy acts as the articulation of PC's privacy practices and standards regarding the collection, use, and disclosure of personal information and personal health information in the course of its operation. This document is intended to guide all the staff and third parties who are given access to personal information in PC's possession.
  • As used in this policy, the term 'personal information' is inclusive of personal health information, unless the latter term is used exclusively. Hence, in that case, it applies only to personal health information.

 2.  Policy Appication

  • This policy is applied to all the staff at all levels and locations in PC.
  • Besides, our company requires any individuals or third parties who collect, use, or disclose personal information on behalf of the organization to adhere to the provision of this policy in relation to the respective work.

 3.   Policy Details

      3.1.  PC's Property

  • Any records referred to in the document as personal or health information are and will remain as the property of PC. Staff is required to maintain the privacy and confidentiality of all records in any or all formats both while acting as active staff members or after they leave the Clinic.
  • 3.2.  Personal and health information collected

  • PC collects the minimum and related information needed in order to implement the healthcare process such as patient management, double-checking patients' information, etc. Information that is included:
      -   Contact and identification information, such as name, address, phone number, and email address.
      -   Doctor's diagnosis.
      -   Personal information that was not requested but voluntarily given to us.
      -   Other personal information is used for purposes that a reasonable person would consider appropriate in the circumstances.

   3.3.  Privacy and Confidentiality Principles

    3.3.1   Accountability

-  PC is responsible for keeping the customer's information under control. This will be acted by practices and procedures to carry out the policy:

  • Implementing procedures to protect personal information or to receive and respond to the compilation of personal information from customers.
  • Making sure all the staff are trained about Privacy and Confidentiality policy and practices.

    3.3.2  Limit use

-The collection of personal information will be limited to that which is necessary for the purposes identified by PC. Information will be collected based on fair and lawful means.

    3.3.3  Identifying Purpose

PC will only collect information used for purposes not previously identified. The purpose will be decided before collecting the information. The clinic is obligated to communicate the purpose to each individual and obtain their consent before using and storing the information.

   3.4.  Thrid parties

Shared personal information with third parties in connection with the needed information to implement the process of the clinic. This information will be secured carefully and only allowed to share with the right third-party sites or platforms.
